IDSP Diseases List: Conditions Under Surveillance
Officially, IHIP tracks 33 diseases, up from 18 under the earlier IDSP system. We could not confirm the current 33-item list from an official source, so this page shows what IDSP has published and labels it honestly.
- Diseases on IHIP
- 33, up from 18 (PIB, 5 Apr 2021)

- Malaria, Tuberculosis, Typhoid
"Which diseases come under IDSP?" is one of the most searched questions about the programme, and also one where many websites copy a list without a source. We would rather be clear. The Health Ministry's press release of 5 April 2021 says IHIP tracks 33 diseases, up from 18 under the earlier IDSP system. NCDC's newsletter (Oct to Dec 2023) says IDSP's December 2016 re-prioritisation identified 33 priority health conditions. WHO India described the platform as covering "more than 30 diseases".
What we could not find is an official page or manual that names all 33 conditions. Until we can, we will not print a 33-item list. Below you will find the lists IDSP did publish for its older paper forms, clearly marked as historical, plus a practical way to get the exact current list for your state. This is an independent guide, not an NCDC or MoHFW page.

Journal articles describe "33+" conditions, and a 2022 paper has a table of key syndromes and diseases under IHIP, but we could not read its full text. If you see a confident 33-item list online, ask where it came from.
Published list 1: legacy P form diseases (historical)
The IDSP "Training Manual for Paramedical Staff For Hospital Based Disease Surveillance" gives the tally sheet for the old paper P form. This is the doctor-level list from the paper era. It is historical, and it is not presented as the current IHIP list.
| No. | Disease or condition (as written) |
|---|---|
| 1 | Acute Diarrheal Disease (including acute gastroenteritis) |
| 2 | Bacillary Dysentery |
| 3 | Viral hepatitis |
| 4 | Enteric Fever |
| 5 | Malaria |
| 6 | Dengue/DHF/DSS |
| 7 | Chikungunya |
| 8 | Acute Encephalitis syndrome (AES) |
| 9 | Meningitis |
| 10 | Measles |
| 11 | Diphtheria |
| 12 | Pertussis |
| 13 | Chicken pox |
| 14 | Fever of unknown origin (PUO) |
| 15 | Acute Respiratory Infection (ARI)/influenza like illness (ILI) |
| 16 | Pneumonia |
| 17 | Leptospirosis |
| 18 | Acute Flaccid Paralysis < 15 years of age |
| 19 | Dog Bite |
| 20 | Snake Bite |
| 21 | Other |
The manual also says states may add state-specific diseases. So even in the paper era, the list in one state could be longer than in another. More on the doctor's form: IHIP P form guide.
Published list 2: legacy S form syndromes (historical)
The health worker's paper Form S did not list diseases at all. It listed syndromes, because a health worker reports symptoms, not diagnoses. The published Form S had these groups:
- Fever < 7 days: only fever; with rash; with bleeding; with daze, semiconsciousness or unconsciousness
- Fever > 7 days (under 3 weeks; over 3 weeks)
- Cough with or without fever
- Loose watery stools of less than 2 weeks duration (with some or much dehydration; with no dehydration; with blood in stool)
- Jaundice cases of less than 4 weeks duration
- Acute flaccid paralysis cases in less than 15 years of age
- Unusual symptoms leading to death or hospitalization that do not fit into the above
This is also historical. The current IHIP S form may group these differently. See the IHIP S form guide for how health workers use it.
Published list 3: legacy lab form L1 (historical)
The old IDSP Form Guidelines gave peripheral labs at PHCs and CHCs a short lab form, L1, covering Malaria, Tuberculosis and Typhoid (Typhi Dot once introduced). Larger labs used L2 and L3 forms. On IHIP, labs now use a single L form with a "Pathogen" field that appears when a result is positive. See the IHIP L form guide.
How to get the exact current list for your state
If you need the list for a report, exam answer or training session, these steps give you a reliable answer instead of a copied one:
- Ask your District Surveillance Unit (DSU) or DSO for the current list of diseases and syndromes used in your state on IHIP.
- If you have an IHIP ID, check the disease or syndrome dropdown on your own form on the official IHIP portal. That is what your state actually uses.
- Check whether your state has added state-specific conditions, as IDSP allowed in the paper era.
- For written work, cite the source and date of the list you use, for example "State IDSP cell, October 2026".
- If you only need the national count, cite the official figure: 33 diseases on IHIP, up from 18 (PIB, 5 April 2021).
Other programmes that report on IHIP
Part of the confusion about "IDSP diseases" comes from the fact that IHIP is a platform for more than IDSP. NCDC's 2023 newsletter lists other programmes on IHIP: the National Programme on Climate Change and Human Health (NPCCHH), the National Malaria Elimination Programme, the National One Health Programme for Prevention and Control of Zoonoses, and the Guinea Worm Eradication Programme. It also notes that NVBDCP dengue reporting was still migrating. A 2023 journal article (IJFCM) also mentions special modules for COVID-19 and INSACOG. So a disease you see in an IHIP module may belong to another programme, not to the IDSP S, P and L forms. Background on the programme itself: IDSP explained.
Why the list matters for outbreak detection
The list is not just for tallies. Each condition has a case definition and, in older IDSP training material, a trigger level that tells staff when to suspect an outbreak. Examples from the 2008-era Module 5: for dengue, a cluster of 2 probable cases or a single DHF or DSS case; for JE, more than 4 cases from a PHC in a week; for AFP, "Even a single case of AFP reported will trigger the outbreak investigation"; for plague, rat fall. These are older guidance, so check current state instructions. How alerts move from there is covered in outbreak reporting in IDSP.
Frequently asked questions
How many diseases are covered under IDSP on IHIP?
The Health Ministry's press release of 5 April 2021 says IHIP tracks 33 diseases, up from 18 under the earlier IDSP system. Separately, NCDC says IDSP's December 2016 re-prioritisation identified 33 priority health conditions. WHO India described the platform as covering more than 30 diseases.
Where can I find the official list of 33 IHIP diseases?
We could not find an official page or manual that names all 33 conditions. Ask your District Surveillance Unit or DSO for the list used in your state, or check the dropdown on your own form on the official IHIP portal. Be careful with unsourced 33-item lists copied across websites.
Which diseases were on the IDSP P form?
The published legacy P form tally sheet listed 21 rows: acute diarrhoeal disease, bacillary dysentery, viral hepatitis, enteric fever, malaria, dengue/DHF/DSS, chikungunya, AES, meningitis, measles, diphtheria, pertussis, chicken pox, PUO, ARI/ILI, pneumonia, leptospirosis, AFP under 15, dog bite, snake bite and other. This is historical, not the current IHIP list.
Is the 18-disease list the same as the old P form list?
We cannot say. The press release gives only the count of 18 for the earlier system, not the names. The published legacy P form tally has 21 rows including "Other", and states could add their own diseases, so the two numbers do not map neatly. Avoid treating either as a definitive current list.
Why does the S form list syndromes and not diseases?
Health workers at sub-centres report what they see, such as fever with rash or loose watery stools, without a doctor's diagnosis. IDSP calls this syndromic surveillance: "surveillance of diseases based on the presenting symptom/s". Diagnoses are recorded on the P form by clinicians, and confirmations on the L form by labs.
Can states add diseases to the IDSP list?
Yes, in the paper era. The IDSP paramedical training manual says states may add state-specific diseases to the P form list. Whether and how each state adds conditions on IHIP today is something to confirm with your State or District Surveillance Unit.
