S, P and L Forms Explained: Which IDSP Form Do You Fill?
IDSP has three reporting forms: S for suspected cases, P for presumptive cases and L for lab confirmed cases. Your job decides your form. This page compares them and shows how one case moves through all three.
- S form
- Suspected Cases, by health workers (sub-centre)
- P form
- Presumptive Cases, by clinicians (PHC, CHC, hospitals)
- L form
- Laboratory Confirmed Cases, by lab staff
- Source of names
- NCDC IDSP page
- Link on IHIP
- P form cases needing tests go to the linked L form user
- Reporting rates 2025-26
- S 78.31%, P 82.01%, L 83.40% (NCDC)
New staff often ask the same question in their first week: "Which form is mine?" The answer depends on where you work and what you do, not on the disease. NCDC describes the IDSP forms as "S" (Suspected Cases), "P" (Presumptive Cases) and "L" (laboratory Confirmed Cases), filled by health workers, clinicians and laboratory staff respectively. On IHIP, each user ID is tied to one facility or lab, and the manuals link each form to a user type.
If you just want a quick answer, use the form picker at the top of this page: choose your role and workplace, and it points you to the matching guide. Below, we explain the logic in full, with a comparison table, the case flow, the older paper forms and the mistakes that cause double counting. This is an independent guide; all real entry happens on the official IHIP portal.
The three forms side by side
| Point | S form | P form | L form |
|---|---|---|---|
| Official name | Suspected Cases | Presumptive Cases | Laboratory Confirmed Cases (IHIP: "Laboratory Cases Form") |
| Filled by | Health workers | Clinicians (Medical Officers, doctors) | Laboratory staff |
| IHIP user type | Health Sub Centre users | Health facility users: PHC, CHC, SDH, DH, medical college, private hospitals | Internal labs (PHC, CHC, SDH) and external labs (DPHL, medical college, private or reference) |
| Basis of the entry | Presenting symptoms (syndrome) | Typical history and clinical examination by a medical officer | Positive laboratory identification |
| Entry channel | Desktop or IHIP mobile app | Portal (app use not confirmed) | Portal (app use not confirmed) |
| Reporting rate, 1 Apr 2025 to 31 Mar 2026 | 78.31% | 82.01% | 83.40% |
| Reporting units, 2025 | 184,895 | 43,180 | 36,414 |
Names and user types come from the NCDC IDSP page and the IHIP manuals; rates and unit counts come from the NCDC IDSP page.
How one case moves from S to P to L
The three forms are not three separate systems. They are three levels of certainty about the same kind of event. IDSP training material defines them as: suspect case, identified by presenting symptoms; presumptive or probable case, a "diagnosis made on the typical history and clinical examination by a medical officer"; confirmed case, a "clinical diagnosis with positive laboratory identification".
Here is how that looks with a generic example (no real patient):
- An ANM on a home visit finds a person with fever and rash. She records it on the S form as a syndrome.
- The person attends the PHC OPD. The Medical Officer examines them and records a presumptive diagnosis on the P form.
- If a test is needed, IHIP auto-forwards the P form case to the linked L form user, according to the IHIP manuals.
- The lab enters the result on the L form. If the result is "Positive", a "Pathogen" field appears.
- The DSU sees all three levels together and can spot a cluster early.
In our understanding, a case does not have to pass through all three: only P form cases that need a test are forwarded to a lab, and S form entries are syndromes rather than diagnoses. The forms complement each other.
S form in brief
The S form is for symptoms only. On the old paper form, its header read "(To be filled by Health Worker, Village Volunteer, Non-formal Practitioners)" and it counted cases and deaths in under-5 and over-5 columns across seven syndrome groups, from short fever to "unusual symptoms leading to death or hospitalization". On IHIP the S form sits with Health Sub Centre users and can be filled from the IHIP Android app. Full details: IHIP S form guide.
P form in brief
The P form records what a doctor thinks the illness is. The IHIP P form manual lists three kinds of entry: Presumptive Cases, Death and Aggregate. Aggregate entry needs DSO permission. The manual expects data to be entered every day. The older paper P form tally listed diseases such as acute diarrhoeal disease, malaria, dengue, measles, pneumonia, dog bite and snake bite, and states could add their own. Full details: IHIP P form guide.
L form in brief
The L form records lab results. Its menu, as given in the IHIP lab manuals, includes "Add Patient Record", "Line Listing", "Edit", "Update", "Save", "Summary", "Print" and "Submit", and Lab Details can be "Internal" or "External". The manuals ask labs to enter results within 7 days of testing, and the Date of Onset must be within 30 days before the reporting date. Back-dated entry is discouraged. Full details: IHIP L form guide.
Older paper forms: L1, L2, L3 and W
Before IHIP, IDSP ran on weekly paper forms. The IDSP Form Guidelines (historical) split the lab form into three:
- L1: peripheral labs at PHCs and CHCs. Form L1 covered malaria, tuberculosis and typhoid (Typhi Dot once introduced).
- L2: district public health lab and district or private hospital labs.
- L3: medical college, tertiary and reference labs.
There was also Form W, the "Reporting Format for Water Quality Monitoring". You may still see these names in old registers, training slides and exam questions. On IHIP the lab side is handled through the L form, with internal and external labs. What changed in the move is covered in IHIP vs the old IDSP portal.
Mistakes that cause double counting or gaps
- Doctor fills the S form, or ANM fills the P form. The manuals link each form to a user type (sub-centre, facility, lab), so keep to your own form and the three levels stay meaningful.
- Same patient entered twice on the P form by OPD and IPD staff. Agree inside the facility who enters what.
- Lab result not linked because the lab entered a new record instead of using the forwarded case. Check the forwarded list first.
- Late lab entry beyond the 7-day rule in the manuals.
- Silence instead of reporting. Performance is reviewed week by week on the Monday to Sunday IDSP week; see the reporting schedule.
Frequently asked questions
What is the difference between the S, P and L forms?
The S form records suspected cases from symptoms and is filled by health workers. The P form records presumptive cases diagnosed by a doctor from history and examination. The L form records laboratory confirmed cases and is filled by lab staff. Together they show the same disease picture at three levels of certainty, from early signal to confirmation.
Which IDSP form does an ANM fill?
The health worker at a Health Sub Centre fills the S form. The IHIP manuals map the S form to Health Sub Centre users, who can report from a desktop or the IHIP mobile app. A 2023 journal article (IJFCM) also describes the S-Form as filled by ANMs from Sub Centres.
Which form does a Medical Officer fill?
Medical Officers and other clinicians fill the P form. On IHIP this belongs to health facility users such as PHCs, CHCs, SDHs, district hospitals, medical colleges and private hospitals. The P form manual lists Presumptive Cases, Death and Aggregate entries, with aggregate entry needing DSO permission.
What are L1, L2 and L3 forms?
They are older paper IDSP lab forms. L1 was for peripheral labs at PHCs and CHCs, L2 for the district public health lab and district or private hospital labs, and L3 for medical college, tertiary and reference labs. On IHIP, labs use one L form, with lab details marked Internal or External.
What is Form W in IDSP?
Form W was the "Reporting Format for Water Quality Monitoring" in the older IDSP paper system, described in the IDSP Form Guidelines. It sat alongside the S, P and L forms. It is mainly of historical interest now; ask your district surveillance unit how water quality data is handled in your state today.
Do all cases go through S, P and L?
No. The forms are complementary. A case may appear only on the S form, only on the P form, or on P and L when a test is done. On IHIP, P form cases that need a test are auto-forwarded to the linked L form user, so the lab can add the result to the same case.
